SUPPORT AT HOME
Support At Home (My Aged Care)
What is Support at Home?
Support at Home is our mobile physiotherapy service designed to provide expert clinical care within the familiarity of your own residence. This service is ideal for those who may find it challenging to attend our Pimpama clinic due to mobility limitations, recent surgery, or long-term health conditions.

By bringing our expertise to you, we can focus on "real-world" rehabilitation. Our physiotherapists work with you to improve your mobility and safety within your actual living environment - addressing specific challenges like navigating your own stairs, kitchen, or bathroom - to ensure your recovery is practical and directly improves your daily life.

Eligibility for Support At Home Program
Understanding the eligibility criteria for Department of Veterans’ Affairs (DVA) physiotherapy services is essential for veterans seeking rehabilitation and support for their health and wellness needs. At Pimpama Physio, we aim to provide clarity on who qualifies for DVA physiotherapy services and how veterans can access the care they deserve.

Eligibility Criteria:
01
Age Requirements:
-
You are 65 years of age or older.
-
For Aboriginal or Torres Strait Islander people, the age requirement is 50 years of age or older
02
Assessed Clinical Need:
You must be assessed by My Aged Care as having a functional need for help to continue living safely and independently at home. This often includes requiring assistance with mobility, personal care, or managing health conditions (like vertigo or post-surgical recovery).

03
Residency Status:You must be an Australian citizen, a permanent resident, or hold a specifically eligible visa (e.g., a protected Special Category visa).
04
Living Situation:
The program is specifically for individuals still living in a private residence. You are generally not eligible if you already live in a residential aged care facility that provides these services.
05
Program Exclusivity:
You must not be receiving equivalent support through another government-funded program, such as the NDIS (which is typically for those under 65).
There are also two short-term pathways:
-
Restorative Care Pathway: You may be eligible for immediate, intensive support (up to 16 weeks) if you are recovering from a hospital stay or a sudden decline in function and have a high potential to regain independence.
-
End-of-Life Pathway: Higher levels of funding are available for those assessed as being in their final three months of life to ensure they can remain at home.

The steps to accessing Support At Home services:

01. Register & Check Eligibility
The first step is to contact My Aged Care. You can use their online eligibility checker or call 1800 200 422. You will need your Medicare card ready. They will ask about your current health, mobility, and how you are managing daily tasks at home.

02. The Home Assessment

If eligible, a trained assessor will visit you at home to conduct an Integrated Assessment. They look at your physical needs and safety risks. Following this, you will receive a Notice of Decision letter confirming your classification level (1–8) and your approved budget for clinical services like physiotherapy.

03. Choose Your Physio

Once your funding is allocated, you simply choose a registered Support at Home provider and name Pimpama Physiotherapy as your preferred clinical provider. We then work with your provider to set up a service agreement and begin your home-based treatments - often with $0 out-of-pocket cost for clinical care.

Support at Home physiotherapy services are available to eligible community members who require clinical care within their own residence due to mobility challenges, post-operative recovery, or long-term health conditions. By understanding the eligibility criteria and accessing the necessary funding pathways through your provider, you can access high-quality physiotherapy care to support your independence and well-being without leaving your house. At Pimpama Physiotherapy, we are dedicated to serving the needs of our local community and providing our home-based patients with the professional support and clinical expertise they deserve.
